
Insurance
Our clinic is in-network with most major health insurance providers. Typically services are covered fully or partially by medical insurance. To better understand your coverage, please contact your insurance provider the following questions:
- Is approval or a referral required from my primary care physician?
- What is my deductible, and has it been met?
- What is my copay for the requested service?
Forms
All clients will receive an email link to sign required forms, answer health questions, and verify insurance details prior to your appointment.
Our Release of Information form is available here and can be submitted prior to your first appointment. Submitting your information electronically helps ensure a seamless visit. Paper forms are also available at all clinic locations.
Payment
We accept various forms of payment, including insurance, Healthcare Savings Accounts (HSAs), cash, check, and all major credit cards. Patients receive monthly statements outlining account activity and balances. Payment options include:
- In Person or by Telephone: Payments can be made to the receptionist or clinician using cash, check, or credit card (Amex, Visa, MasterCard, Discover). You can also call us at 651-243-4499 Monday–Friday, 9:00 AM to 5:00 PM, to make payments over the phone.
